The origins of the Miner vary depending on which film your watching.

If its the original My Bloody Valentine, then it could be either Harry Warden or Axel Palmer who had seen his father murdered by Harry Warden and went insane and took on the persona.

If you're watching My Bloody Valentine 3D then it is either Harry Warden or Tom Hanniger who screwed up something in the mine causing Harry and others to be trapped in the mine. While there Harry murdered those trapped with him to save air. When rescued he was in a coma. A year later he awakens to go on a murder spree in which Tom is severely, mentally disturbed and then slips into mental lapses where he himself becomes the Miner.
